Your Rehabilitation Team

Nambour Selangor Private Hospital’s rehabilitation programs are managed by a team of health care professionals to help you achieve your rehabilitation goals and assist in your recovery. For some, this may be getting back to work, back to driving or back to your favourite leisure activity. For others, it may simply be to help you live as independently as possible in the community. Which specialists are involved in your rehabilitation will depend upon your individual needs but may include:

Rehabilitation Specialist / Geriatrician

The rehabilitation specialist or geriatric medicine specialist has responsibility for monitoring and evaluating medical and rehabilitation care. They liaise with a patient’s surgeon or physician, GP and other specialists to ensure effective continuity of medical care. 

Physiotherapy

  • Improving functional mobility, balance and coordination
  • Improving strength and flexibility after surgery or an injury
  • Pain management
  • Develop fitness and physical abilities
  • Managing chronic health conditions

Occupational Therapy

  • Regaining independence
  • Making everyday activities safer and easier
  • Falls prevention and energy conservation
  • Improved sleep and stress management
  • Cognitive assessment and memory strategies

Exercise Physiology

  • Chronic disease prevention and management
  • Improving self-confidence, health and functional mobility
  • Strength and conditioning
  • Improving quality of life using ‘exercise as medicine’
  • Exercise prescription that is individualised and achievable

Nursing

  • Continuous assessment and management of patient status in relation to physical, cognitive, social, emotional and spiritual health and wellbeing
  • Early identification of and escalation of any patient deterioration
  • Coaching and prompting patient independence to achieve goals related to specific nursing therapies relating to continence management, safe medication administration, maintenance of skin integrity as well as oral nutritional/fluid intake
  • Liaising with and working with the multidisciplinary team to support patients to achieve their rehab goals

Psychology (as required)

  • Emotional wellbeing and psychological support
  • Assistance with interpersonal interactions
  • Help adjusting to new circumstances

Speech Pathology (as required)

  • Communication assessment and intervention
  • Swallowing assessment and intervention
  • Speech, voice and fluency therapy

Social Work (as required)

  • Work closely with patient and their family to answer queries
  • Provide emotional support and information
  • Help adjusting to new circumstances
  • Facilitate access to community resources, government services and systems

Dietetics (as required)

  • Chronic disease nutritional management
  • Nutritional assessment, education, advice and support
  • Meal planning and recipe modification

Allied Health Assistant

The allied health assistant coordinates, supervises and assists with individual and group therapy sessions as directed by your therapists.

You, the patient

You the patient, are at the centre of the rehabilitation program, as it is designed to meet your needs. Our philosophy is to maximise independence through goal setting and active participation. Your rehabilitation team meets regularly to monitor your progress and review your goals. You are a vital member of the team and will be encouraged to participate in all aspects of your program.
